Information for Our Patients
It is our optimal goal to provide you and your family with the highest quality of dental care while maintaining a friendly and relaxing environment. In order to keep our standard of care at a level which best serves your dental needs, we ask you to please observe the following guidelines.
Unless prior arrangements have been made, payment is due at the time of service. We accept cash, funded checks, Care Credit, Springstone Patient Financing, Visa or MasterCard debit or credit cards, American Express, and Discovery cards.
We accept most dental insurance; however we do require your co-payment and/or deductible to be paid at the time of service. We give you our best estimate based on the information you and your insurance company provide us, however, we CANNOT guarantee that our estimate will be 100% accurate. Your insurance policy is a contract between you and your insurance company. Anything not paid by your insurance company is your responsibility and we expect your insurance to pay within 60 days of the date of service. Interest charges of 18% will be applied after 60 days. If you are a cash patient we will apply interests charges of 1.5% per month 30 days after the date of service.
There are many times when our patients require urgent or emergency treatment and therefore need an appointment as soon as possible. When patients give advanced notice of their need to cancel a scheduled appointment, this time can be allocated to those patients with immediate needs. Bearing this in mind, our office requires a minimum of 48 hours notice to cancel or reschedule and appointment. Otherwise there will be a $45 dollar charge,> which is, in full, the patient’s responsibility. Charges must be paid before rescheduling an appointment.
